Chelsea began her Health and Social Care career in 2021 while completing a Master’s in Health Promotion. She joined Liaise in late 2024 and is driven to improve health and well-being through compassionate, informed, and inclusive care that meets each person’s needs.

Our Referral Process

To guide you through every stage of the referral journey, we’ve created a clear roadmap that takes you from initial contact all the way to a successful placement.
Enquiry Received

We will respond within 24 hours of referral

Assesment

We will arrange an assessment to fully understand the needs of the individual within 5 working days

Invited to View Homes

We encourage you to visit our existing homes or new homes

Sending an Offer

Following the assessment, we will make an offer within 10 working days

4
Transitions

We will begin co-produced transition planning once a placement offer has been accepted

Move in

To support a successful transition, we’ll offer home visits and stays and we'll review the placement at frequent intervals within the first three months
