Referring a person in need of support to our homes is easy. Simply call our ‘Referral Hub’ 0330 500 5052 or follow our streamlined referral process by completing our referral form and submitting it to our dedicated team of Referral Coordinators. We’ll review the details promptly and collaborate with you to ensure we find the best possible home for the person in need of support.

Theresa joined Liaise in February 2025 with over 30 years’ experience in social care, across children’s and adults’ services. She has held senior roles in operations, referrals, and business development, including Head of Referrals and Head of Business Development. Her expertise in placements and service development supports our continued growth and commitment to quality care.
