Theresa joined Liaise in February 2025 with over 30 years’ experience in social care, across children’s and adults’ services. She has held senior roles in operations, referrals, and business development, including Head of Referrals and Head of Business Development. Her expertise in placements and service development supports our continued growth and commitment to quality care.

Chelsea began her career in Health and Social Care in 2021, working for a local authority while completing a Master’s in Health Promotion. She joined Liaise in late 2024 and is driven by the opportunity to enhance health and wellbeing through compassionate, informed, and inclusive care, ensuring each placement is tailored to individual needs.
Our Referral Process
We will respond within 24 hours of referral
We will arrange an assessment to fully understand the needs of the individual within 5 working days
We encourage you to visit our existing homes or new homes
Following the assessment, we will make an offer within 10 working days
We will begin co-produced transition planning once a placement offer has been accepted
To support a successful transition, we’ll offer home visits and stays and we'll review the placement at frequent intervals within the first three months
