Supporting Mohammed in attending the mosque was an important and meaningful moment for him. Being able to actively take part in his religious practice allowed him to connect with his faith, honour his cultural identity, and engage in something deeply personal to him.
With the right support in place, Mohammad was able to participate confidently and independently, ensuring his spiritual and cultural needs were respected and valued. Experiences like this are a powerful reflection of person-centred support in action; promoting equality, dignity and the freedom to live a life that feels authentic and meaningful.
This support led to several meaningful outcomes from our Outcomes Pathway Tool:
- Increased community participation, as Mohammed was able to reconnect with an important part of his cultural life.
- Strengthened personal identity and wellbeing, with Mohammed expressing pride and comfort in being able to practise his faith in a familiar environment.
- Improved confidence, as he completed the journey and visit with reduced anxiety and greater independence.
A huge thank you to the team who made this possible. Their thoughtful, respectful approach ensures the people we support can continue to practise their beliefs and stay connected to what matters most to them.
