Here at Liaise, We Know That People Living With Mental Health Conditions Face Day-to-Day Challenges That Can Impact Their Ability to Live Independently

We offer person-centred supported living and residential housing solutions for people that require extra help in managing their mental health so that they can live their best life.

We support people with a wide variety of mental health needs including bipolar disorder, depression and obsessive-compulsive disorder. We create a detailed, highly-personalised support plan for each individual that takes into consideration not only their support needs but their goals and preferences as well.

In order for our services to provide support, these and other mental health conditions must be secondary to the person’s learning or mental health needs.

We encourage each individual to enjoy as much independence as possible with the reassurance that extra support is always available. We provide dedicated 24/7 support in all of our supported living and residential housing solutions.

Through self-development and a supportive approach from our staff, people living with mental health conditions can progress at their own pace and move towards increased independence.

Our Residential and Supported Living Services

Since we support a wide range of ages and abilities, each Liaise home has its own distinct character and personality. We work with the people we support and their families to help them find accommodation options that match their needs and wishes.

We believe that a supportive environment is essential for helping people overcome mental health challenges. We foster calm and relaxing environments that empower people to build confidence and grow their everyday life skills with the appropriate safeguards in place to support people living with mental health conditions.

Our approach is guided by the principles of “person-centredness”. This means the support we provide is always personally tailored to the specific needs and wishes of each person we support.

We Offer a Wide Range of Services and Activities Including:

  • Daily activity programmes tailored to the capabilities of each individual
  • Support in finding employment, education or volunteering opportunities, where appropriate
  • Through opportunities and with guidance, we help each person to develop relationships that are meaningful to them
  • Outings and trips into the local community
  • Regular social events, both in home and in the community
  • Support that enables the growth of practical home focused skills such as cooking, housework and decorating.
  • Each person is supported within a person-centred framework, to develop their community skills such as shopping, banking and the management of health appointments
  • Specialised positive support for anxieties or behavioural presentations that challenge the individual or those around them

Here at Liaise, we are very proud of the high levels of support we provide. Our commitment to quality is reflected in our outstanding outcomes and sector-leading quality ratings. We are proud to have achieved an outstanding CQC rating not once, but twice in a row for one of our services. Two of our other services have received an outstanding outcome in one of the five areas as well.

The Liaise Approach

Due to the wide range of wonderful personalities that live there, each of our homes is unique. To help find every person their ideal match, we always consider the goals, interests and ambitions of each individual. This allows us to provide bespoke guidance to every person as they are choosing a home that compliments their lifestyle.

Through many years of experience, we know that generalised support plans simply don’t work. By recognising that people are not all the same and celebrating these differences, we create individual support plans tailored to the unique personality of each person we support.

We work in close partnership with local authorities and healthcare associations and we can arrange introductions, visits or assessments for the people we support. Our dedicated staff receive regular training on the best practices for supporting people living with mental health conditions with a focus on risk management, medication support and independent living skills.

All support plans are reviewed in partnership with the individual to ensure they are meeting expectations and outcomes. Where appropriate, we welcome families, advocates and friends to contribute to this review. This allows us to create a bespoke care plan for each individual that best meets their needs.
