Liaise is proud to have been awarded a NorCA Star by the Norfolk Care Association, recognising the exceptional advocacy, professionalism and dedication shown by our East Anglia team over an extended period of support.
The award highlights a powerful example of person‑led care in practice and reflects the team’s unwavering commitment to improving the quality of life for an individual with highly complex needs.
Seeing the Person Behind the Needs
When the Liaise team first began supporting this individual, they inherited a situation shaped by long‑standing assumptions about what was and was not possible. The team took time to truly understand the person, building trust and identifying opportunities to enhance their day‑to‑day life through further assessment and review.
Through ongoing engagement with external professionals and a strong belief in the importance of regular reassessment, the team advocated for a fresh look at the individual’s needs. As a result, appropriate equipment was introduced, including a hoist, sling and wheelchair, enabling the individual to spend time outside of their room and engage more fully with the world around them.
Advocacy Beyond the Home
More recently, the individual experienced a significant decline in their health and required an extended stay in the hospital. Throughout this period, the Liaise team maintained a consistent and compassionate presence, ensuring continuity of care and providing vital insight into the individual’s communication, preferences and needs.
By working closely and collaboratively with clinical teams, the Liaise staff supported informed decision‑making and helped ensure that the individual’s best interests remained central at every stage. Their detailed understanding of the person proved invaluable during a complex and emotionally challenging time.
Preparing for a Safe and Supported Return Home
While advocacy continued in the hospital setting, the wider Liaise team remained focused on preparing for the individual’s return home. The individual’s living space was thoughtfully redesigned to reflect their needs, preferences and wellbeing, creating a safe, welcoming and truly person‑centred environment.
This transformation ensured that, upon discharge, the individual returned not only to familiar support but to a space that better reflected who they are and what they need to thrive.
Recognition of Dedicated Care
The NorCA Star is a testament to the collective dedication, perseverance and compassion shown by Beverley and the entire Liaise team. It recognises their determination to keep advocating, their belief in the potential of the people they support, and their commitment to delivering care that goes beyond meeting needs to truly improving lives.
