Jean has faced mobility challenges that previously limited her independence and participation in daily life. With structured guidance, encouragement, and clear goals, she has made significant progress in mobility, confidence, and engagement – a strong example of mobility support outcomes in action.
Starting the Journey with Liaise Outcomes Support
After a hospital stay, Jean returned home requiring support to move safely. Her care team worked alongside a physiotherapist to create a gradual, step-by-step plan. Jean began using a walking frame to rebuild strength, and with consistent practice, she steadily gained confidence in her mobility.
Jean’s initial steps on her mobility journey were documented in her earlier blog, showing how small achievements laid the foundation for greater independence.
Gaining Confidence and Independence
Jean progressed from using a wheelchair for longer distances to walking short distances independently. She was able to attend social events and day centre activities, enjoying experiences she couldn’t access before. One particularly joyful moment came when Jean took part in a Halloween celebration, dressed as a witch, and danced without needing her walking frame or wheelchair.

Her milestones reflect growth across key areas in Liaise’s Outcomes Pathway Tool, including:
- Physical Health: Improved walking, balance, and strength
- Growing Independence: Taking part in routines and community activities on her own
- Feeling Good: Enjoying social events and celebrations boosts her confidence and enjoyment
- Mental Health: Achieving goals and building self-esteem support emotional well-being
Jean’s progress aligns with our Outcomes Pathway Tool, showing measurable growth in physical health, growing independence, feeling good, and mental health through structured mobility support outcomes.

Celebrating Jean’s Achievements
Jean’s story demonstrates how targeted support and achievable goals can create meaningful mobility outcomes. Her journey from being bed-bound to walking and participating in activities she enjoys illustrates how structured guidance and encouragement enhance independence, confidence, and well-being.
We are proud of Jean’s achievements and look forward to seeing her continue to build on her progress with the continued support of her team.
